Patient Terms of Use and Telehealth Consent
For patients using telehealth and ordering through the platform.
Draft — not yet cleared for use. This wording was written for MedGrid but has not been reviewed by counsel or the medical director. Do not use it to obtain patient consent until it has been signed off. The page is noindex meanwhile.
Effective Date: September 30, 2026
These Patient Terms of Use and Telehealth Consent ("Patient Terms") apply if you use telehealth or place an order through a MedGrid-connected clinic's use of the MedGrid platform. They work alongside two other documents: our general Terms & Conditions of Use, which governs use of the MedGrid platform generally, and the Informed Consent for Telehealth Services, which is the specific consent you will be asked to acknowledge before each telehealth encounter and covers what that encounter involves, its risks and limits. Where these Patient Terms and the Informed Consent both address telehealth, the Informed Consent controls for anything specific to a given encounter.
1. Who You Are Agreeing With
Your care is provided by the licensed clinic and clinician you are a patient of, not by MedGrid. MedGrid provides the platform your clinic uses to run the encounter, keep the record, route prescriptions, and process orders. These Patient Terms govern your use of that platform; they do not make MedGrid your healthcare provider.
2. Eligibility
You must be at least 18 years old, or the parent or legal guardian of the patient, to use these features. You must provide accurate identity, contact, location and health information — your provider's ability to treat you safely and lawfully depends on it, including for the state-licensing reasons described in the Informed Consent for Telehealth Services.
3. Creating and Using an Account
You are responsible for keeping your login credentials confidential and for activity that happens under your account. Tell your clinic promptly if you believe your account has been accessed without your permission.
4. Telehealth Encounters
Before your first telehealth encounter, you will be asked to review and accept the Informed Consent for Telehealth Services, which explains what the encounter involves, what it can and cannot do, and the risks and limits you should understand first. You may withdraw that consent at any time, as described there.
5. Ordering, Payment and Shipping
If your provider prescribes a product, your order is placed through the platform and routed to a pharmacy licensed to dispense to you where you are located. Payment is processed by our third-party payment processors; we do not store your complete card number. Shipping timelines depend on the pharmacy or vendor fulfilling your order. Our Return and Refund Policy governs returns, damaged or defective products, and order errors.
6. Communications
By providing your phone number or email address, you consent to receive communications from your clinic and from MedGrid related to your care, your orders and your account, including appointment reminders and order updates. Marketing communications, where sent, will include a way to opt out.
7. Prohibited Use
You may not use these features to provide false identity, location or health information; to obtain a prescription or product for someone other than yourself without authorization; to attempt to circumvent a provider's clinical judgment; or in any way that violates law or the rights of others.
8. Disclaimers and Limitation of Liability
MedGrid provides the platform "as is" and does not practice medicine, does not direct clinical decisions, and does not guarantee any treatment outcome. To the fullest extent permitted by law, MedGrid's liability in connection with your use of the platform is limited as set out in our Terms & Conditions of Use. Nothing in these Patient Terms limits any liability that cannot lawfully be limited, including for your clinic's or clinician's own professional negligence, which these Patient Terms do not purport to address.
9. Disputes, Governing Law and Changes
Any dispute arising from your use of the platform is governed by the Disputes, Arbitration, and Applicable Law provisions of our Terms & Conditions of Use, which these Patient Terms incorporate by reference. We may update these Patient Terms from time to time; we will post the updated version with a revised effective date, and your continued use of the platform after that date constitutes acceptance of the update.
