The First 30 Days of an MSO Build: What Happens, and in What Order

A compliant clinic is built in a sequence, with the legal setup first and the clinical and operational pieces running in parallel behind it. Here is the order, and why it matters.

Key Highlights

  • The build runs in a set order: jurisdiction and service review, entities and agreements, medical director connection, insurance and banking, protocols and systems, then staff and launch readiness.
  • Most clinics are live in about 30 days, with some states, New York among them, taking longer.
  • Many clinics arrive with some of this half-built, and the work is rebuilding underneath a working business without closing it.
  • The same sequence applies to telehealth and in-person clinics.

Day One: The Call

The build starts with a short conversation about what the clinic treats, where it operates and what already exists. We map the state's rules against the service menu and tell the owner which structure is required, what it costs and what has to change.

Week One: Connect and Form

A licensed physician is connected to the clinic, matched to the services it offers, with NPI and licensure verified against the categories on the menu. In parallel the entities are formed: name clearance and state filing, articles, EIN and bank account, with the professional entity set up where the state requires one.

Weeks One and Two: Agreements, Insurance, Protocols

The management services agreement, delegation terms and standing orders are drafted, executed and filed between the practice and the management company. Malpractice and liability coverage is placed, banking is opened in the right structure and payroll points at the right entity. Protocol packs are loaded, consents attached, and the EHR and vendor or pharmacy accounts are configured.

Weeks Two and Three: Staff and Launch Readiness

Staff are invited and credentialed, the clearance screening workflow is trained, and patient-facing copy is checked against the state's advertising rules. This is when a clinic that has been reviewed end to end can treat its first patient with the file to back it.

Ongoing: Live and Maintained

After launch the work becomes maintenance: chart review on the calendar, renewals, director continuity, and revisions when state rules move. A clinic that treats the launch as the finish line is the one that ends up rebuilding later.

If You Are Already Open

Many clinics come in with an entity in the wrong form, a director on a handshake, insurance bound to the wrong name, or protocols copied from a course. We rebuild the structure underneath a working business without shutting it down, and tell you on the first call which steps you can skip. General information, not legal advice.

MedGrid MSO provides management services to independently owned practices; it does not practise medicine and does not direct clinical decisions. Requirements vary by state and by service, and change over time — nothing on this page is legal or medical advice. Talk to us or your own counsel before making a structural decision for your clinic.
